About CSMR
 Center for Sports Medicine and Rehabilitation
offers a full range of rehabilitation services, including
physical therapy, occupational therapy, speech and
pediatric therapy, and athletic training services.
Our staff includes 15 physical therapists,
6 occupational therapists, a speech therapist, 4 physical
therapy assistants and 14 certified athletic trainers.
Patients at CSMR are seen with a doctor's
referral. Most insurance programs cover fees for therapy
services.
CSMR is
a service of Northfield
Hospital, providing services at
two convenient locations. Besides the fully equipped center
at 1301 Jefferson Road in Northfield, CSMR now has a separate
facility in Lakeville. It shares space at Train America,
located next to Northfield Hospital's FamilyHealth
Medical Clinic–Lakeville and
near Lakeville South High School, at the intersection of
County Road 70 and Jacquard Avenue.
