 Birth Certificates
A birth certificate will be completed for your baby prior to discharge. You will be provided the opportunity to proofread this document for errors before the official copy is sent to the Dakota County Courthouse in Apple Valley/Hastings. A copy of your baby's birth certificate is available to you at the courthouse for approximately $15.
You will receive a souvenir birth certificate from the hospital, signed by your doctor.
Upon request the hospital can apply for your baby's Social Security number by checking the appropriate box on the birth certificate information sheet. Your birth announcement will automatically appear in the newspaper and be heard on the radio if you check the appropriate box on the birth certificate information sheet.
