Rehabilitation Services is an outpatient therapy clinic, a service of Northfield Hospital & Clinics with locations in Northfield and Lakeville offering the largest and most comprehensive rehabilitation services between the Twin Cities and Rochester. Scheduling a therapy appointment
We require a written provider's order/prescription to schedule your therapy appointment. Your primary care provider will most likely want to see you for an appointment to evaluate your disorder prior to referring you to therapy. Insurance companies generally accept therapy prescriptions from physicians, nurse practitioners, chiropractors or physician assistants. Once you have the order/prescription for therapy services, you can call the location that is most convenient to you. When you call the clinic, an initial evaluation appointment will be scheduled. We will do our best to accommodate your scheduling needs.
What to expect at your first appointment
Please plan to arrive 15 minutes early to your first therapy visit to allow time to complete registration paperwork. Please bring the following items to your first appointment:
- Current copy of your insurance card (we will make a copy and keep with your records)
- The referral/prescription from your care provider (unless previously faxed to our clinic)
Please plan to be in the clinic between 60-90 minutes for your first visit. During this initial evaluation, the therapist will examine you and establish a plan for further treatment and/or a home exercise program. The treatment plan will include your own personal goals, as well as those of your healthcare provider and therapist. These goals will be reviewed at each visit and will likely include an exercise program that is individually designed to enable your recovery.
What to expect at your next appointment
Follow up visits should take between 30 - 45 minutes. During subsequent visits, it is important for you to keep us informed of any changes that may occur such as change in insurance, a change in address, a change in health care provider or any other pertinent information. Minors (under the age of 18) must be accompanied by a parent or guardian to their first appointment.
Billing information for Rehabilitation Services
Your out-of-pocket expenses for therapy are determined by your individual insurance policy. If you have questions about the cost of therapy, call your insurance company to tell them therapy is billed as a hospital (not a clinic / professional) outpatient service and ask:
- Whether Northfield Hospital is “in network”
- Whether therapy is a covered service under your policy
- What your deductible amount is
- What your co-pay amount is
- What your coinsurance amount will be
- What your yearly maximum out of pocket amount is
Northfield Hospital & Clinics will submit charges for the services you receive to your insurance company who will then determine how much of the bill you will pay and how much they will pay. You will receive a bill from Northfield Hospital & Clinics for the amount you owe.
