Medication Management Services
Our Medication Management program works closely with your primary care provider to ensure your medications are safe, effective, and easy to manage. Taking medications can be complex, and it’s important to understand proper use, potential side effects, and how different treatments interact.
Northfield Hospital and Clinics offers personalized consultations with board-certified pharmacists who are dedicated to helping you get the most from your medications. During your visit, our pharmacists review your entire regimen—from prescriptions to supplements—evaluating how each item works, how they interact, and how they affect your daily life.
We collaborate with both you and your provider to recommend adjustments that support your health and well-being. This may include optimizing doses, simplifying your routine, or identifying cost-saving alternatives.
What to Expect During Your Visit
You’ll meet one-on-one with a pharmacist in a private setting, with phone and virtual visits also available. To get the most from your appointment, please bring:
- All prescription medications (including inhalers, injections, etc.)
- Any infusion medication lists
- Over-the-counter medications
- Vitamins, herbal products, and dietary supplements
Your pharmacist will:
- Review each medication for effectiveness, safety, and possible side effects
- Check for drug interactions or unnecessary medications
- Discuss your health goals and address any concerns
You’ll also receive education on:
- How and when to take each medication (including food considerations)
- Proper use of devices such as inhalers or continuous glucose monitors
- Managing or reducing side effects
- Simplifying your medication routine
- Reducing medication costs
Your Personalized Plan
At the end of your visit, you’ll receive a customized care plan covering all your medications and supplements. If any issues are identified, your pharmacist will work directly with your healthcare providers to recommend appropriate changes to your treatment plan.

Can I bring someone with me to the consultation?
We encourage you to bring a family member, caregiver, or someone who helps with your medications.
Will my insurance cover medication therapy management?
Many insurance health plans cover medication management. If you have questions about coverage, call your insurance company. If you have a Medicare Part D plan, this benefit is called Medication Therapy Management (MTM). Check your prescription drug plan for your specific coverage.
Will you see kids?
Yes! Reviewing every person’s medication is important. We see children as young as 2 years of age.
Do I need a referral?
No referral is needed, but it is preferred that you see your provider first.
Can I get my medication refilled?
This service is not intended for simply getting refills, our pharmacists review y our entire medication regimen – from prescriptions to supplements – evaluating how each item works, how they interact, and how affective they are in your daily life.
