It is now NH+C Jefferson Road Clinic as of Sept. 18, with the same providers and staff continuing to provide excellent care.

Specialists joining NH+C include providers in acupuncture, pharmacology (medication management), podiatry, and sleep medicine. They continue to see patients in this clinic location.

Allina Health now provides audiology and hearing aid services in Apple Valley, Hastings, and Faribault.
Allergy care and shots are available through Allina Health at its Apple Valley and Bloomington locations. Dr. Helm practices at Allina’s Maple Grove and Woodbury locations.
Cardiology care continues at NH+C with Minneapolis Heart Institute.
Minneapolis Heart Institute continues to provide cardiac care through NH+C. Some services change locations.
- Clinic appointments move to Jefferson Road Clinic, from the Northfield Clinic on the hospital campus. To schedule clinic appointments, call 507-646-6300. After Sept. 18, call 507-663-9000.
- Imaging, including echocardiogram and vascular ultrasound, moves to Northfield Hospital. Call Diagnostic Imaging to schedule: 507-646-1143
Minneapolis Heart Institute will no longer offer Teleheart virtual visits in Northfield. There are two options:
- Schedule a clinic appointment at Jefferson Road Clinic. Call 507-663-9000 to schedule
- Set up a Teleheart visit at another MHI location. Call MHI to schedule: 612-863-3900
Prescriptions
Your pharmacy can help manage your existing prescriptions.
- If you have refills available on your prescription, call your pharmacy to refill it as usual.
- If you have no refills, call your pharmacy. They will reach out to the prescribing provider’s care team to discuss refill. Your provider’s care team will determine whether it can be refilled, or if you need to be seen before continuing this medication.
- If you need to be seen before your prescription can be refilled, you will be contacted by your provider’s care team to schedule any follow up you need.

Patient Portal
You can still access MyChart.
You will be able to continue to access your MyChart account to see past messages, and check results for test/labs done before Aug. 28.
If you see other Allina Health providers, you can continue using Allina’s MyChart to communicate with them.
Allina Health shut down MyChart Messaging for Jefferson Road Clinic providers on Tuesday, Sept. 1.

NH+C has a similar patient portal, MyHealth Info.
Your MyChart information does not transfer over to MyHealth Info. You’ll need to set up a MyHealth Info account.
You can set up a MyHealth Info account when your first appointment is scheduled through NH+C. We’ll invite you to do this when you schedule your first appointment with us.
Once your MyHealth Info account is set up, you can use it right away to message your primary care provider, and any NH+C provider you’re scheduled to see.

Electronic Medical Records
You and your provider can still access your Allina Health records.
You will be able to continue to access your Allina Health records.
NH+C is helping patients, providers and staff transition from Allina Health’s Epic system to NH+C’s Meditech Expanse system.
Patients will always be able to see their existing records in Epic; these files will remain, but will not be updated with new information. New information will be added to patient records in Meditech Expanse.
Providers will maintain access to Epic, and continue to see patient records as “read-only” files. Providers won’t have the option to add or make changes to these files.
With two parallel electronic medical records, the back and forth will be a bit more complicated. NH+C is committed to making it as seamless as possible.
Health insurance
Most insurance coverage is comparable to coverage for Allina Health.
BLUE CROSS AND BLUE SHIELD OF MINNESOTA: NH+C is in-network for Blue Cross and Blue Shield of Minnesota insurance. Jefferson Road Clinic is in-network for this insurance as of Sept. 18.
NH+C reviewed insurance products that many people hold in the Northfield area. The vast majority of them are equal from Allina Health to NH+C. There are a few that we're reviewing in more detail to make sure that those patients who have a specific insurance product can continue to get great access with NH+C, and we'll continue to improve that as we go into 2027.
